SPAY / NEUTER
ADDENDUM

This Spay/Neuter Extension Agreement is entered into between Petey and Furends, Inc. and the undersigned adopter regarding the animal identified below (“Subject Animal”). This Agreement supplements and is incorporated into the original Adoption Agreement. All terms of the Adoption Agreement remain in full force and effect except as specifically modified herein.
